If you think that you need support...
Medway Council tries to enable people to live as independently as possible and to make sure the support on offer is the highest quality.
Medway Council Adult Social Care Providers Forum

Transitions
The word “transition” means changing from one part of our lives to another. This could be changing from being a child to being an adult or moving on from school and planning what to do with our lives.
There are lots of ways to get help with this

You and Your Health
We know that if you have a learning disability it’s harder for you to get the help and support you need. So we’ve found a way to write down everything about your health and the care you need. It’s called a Medway Person Held Health Record.
All the people that help look after you can find out everything they need to know about you – and you can wite about yourself as well!
