In 2010-11 Medway took part in a Big Health Check that looked at health services for people with a learning disability in Medway.

The results of all the NHS South East Coast Area Health Checks were put together in one Easy Read report by the Strategic Health Authority.

The Government has set up a Health Observatory to look at the health and lives of people with a learning disability across the country.

The Learning Disability Health Observatory also will collect all of the information from the Big Health Checks and Learning Disability Partnership Board annual reports.

1 - You and Your Health

We know that if you have a learning disability it’s harder for you to get the help and support you need. So we’ve found a way to write down everything about your health and the care you need. It’s called a Medway Person Held Health Record.

All the people that help look after you can find out everything they need to know about you – and you can wite about yourself as well!

3 - Support. Getting the Help You Need

There are many kinds of learning disability so everyone is different – some people need more help and support than others. Lots of people are there to help if you have a learning disability. One of the most important things the Medway Learning Disability People Partnership Board does is to get them all to work together so that you get the help you need.

The Partnership Board is working to make sure that people with learning disabilities have good care.

The Government has said that people with learning disabilities  are one of the most vulnerable groups in society, and should have their life chances improved by having access to care that is:

Arranging your Support

Medway Council may be able to arrange social care for you. You should talk to your Care Manager If you are already receiving care and your needs have changed, or you have a problem.

If you are contacting the Council for the first time about your care you should telephone the Access and Information Team. Their telephone number is 01634 334466.

An Advocate can help you....

Advocacy Partners Speaking Up promotes the rights, voices and choices of people with learning disabilities in Medway.

It provides independent advocacy to enable people to have control over their lives, make decisions, be treated firarly and participate fully in community life.

Transition

The word “transition” means changing from one part of our lives to another. This could be changing from being a child to being an adult or moving on from school and planning what to do with our lives.
